    This study aimed to clarify how behavioral and psychological symptoms of dementia (BPSD) and cognitive function affect the decision-making capacity of persons with Alzheimer's disease (AD) in a real informed consent situation about anti-dementia drug prescriptions. The participants were 76 patients with AD. We used the MacArthur Competence Assessment Tool to assess the capacity for consent to treatment (MacCAT-T). We simultaneously used the Mini-Mental State Examination, Executive Interview, Executive Clock Drawing Task, Logical Memory I of the Wechsler Memory Scale-Revised (LM I), LM II, and Neuropsychiatric Inventory (NPI) to assess cognitive function and psychiatric symptoms. We calculated the correlations between the MacCAT-T scores and the demographic, neuropsychological, and psychiatric variables. Once the univariable correlations were determined, we performed simple linear regression analyses to examine if the regression equations were significant. In the final analyses, we incorporated significant variables into stepwise multiple linear regression analyses to determine the most significant predictors of mental capacity. Age (β = -0.34), anxiety (β = -0.27), and LM I (β = 0.26) were significant predictors of "understanding" (adjusted R 2 = 0.29). LM II (β = 0.39), anxiety (β = -0.29), and education (β = 0.21) were significant predictors of "understanding of alternative treatments" (adjusted R 2 = 0.30). Anxiety (β = -0.36) and age (β = -0.22) were significant predictors of "appreciation" (adjusted R 2 = 0.18). Age (β = -0.31) and anxiety (β = -0.28) were significant predictors of explained variance in "reasoning" (adjusted R 2 = 0.17). Patients with anxiety had lower scores on all five MacCAT-T subscales: "understanding," without 3.8 [SD = 1.2] vs. with 2.6 [SD = 1.1]; "understanding of alternative treatments," without 2.9 [SD = 2.2] vs. with 1.3 [SD = 1.8]; "appreciation," without 2.9 [SD = 1.1] vs. with 1.9 [SD = 1.2]; "reasoning," without 4.0 [SD = 2.0] vs. with 2.7 [SD = 1.7]; and "expressing a choice," without 1.9 [SD = 0.4] vs. with 1.5 [SD = 0.6]. Considering the effects of BPSD, cognitive function, and age/education when assessing consent capacity in persons with AD is important. Reducing anxiety may contribute to improved capacity in persons with AD.

    "delta", a latent variable constructed from cognitive performance and functional status measures, can accurately diagnose dementia. The minimal assessment needed is unknown. We have constructed a delta homolog, "dTEXAS", from Telephone Executive Assessment Scale (TEXAS) items, and validated it in a convenience sample of Japanese persons (n=176). dTEXAS scores correlated strongly with both Instrumental Activities of Daily Living (IADL) (r = -0.86, p < 0.001) and Clinical Dementia Rating Scale (CDR) (r = 0.71, p < 0.001). Constructed independently of their diagnoses, dTEXAS scores accurately distinguished dementia versus controls (area under the receiver operating curve [(AUC; ROC) = 0.92], dementia versus mild cognitive impairment (MCI) (AUC=0.80) and controls versus MCI (AUC= 0.74). These AUCs are higher than those of multiple observed executive measures, as reported recently by Matsuoka et al., 2014. A dTEXAS score of -0.58 best discriminated between dementia versus controls with 90.1% sensitivity and 80.0% specificity.

    The latent variable "delta", can accurately diagnose dementia. Its generalizability across populations is unknown. We constructed a delta homolog ("dT2J") in data collected by the Texas Alzheimer's Research and Care Consortium (TARCC). From this, we calculated a composite d-score "d". We then tested d's generalizability across random subsets of TARCC participants and to a convenience sample of elderly Japanese persons with normal cognition (NC), mild cognitive impairment (MCI), and dementia (AD) (n = 176). dT2J was indicated by Instrumental Activities of Daily Living and psychometric measures. Embedded in this battery were the Mini-Mental Status Examination (MMSE) and an executive clock-drawing task (CLOX). Only MMSE and CLOX were available in both TARCC and the Japanese cohort. Therefore, a second composite variable, "T2J", was constructed solely from the factor loadings of CLOX and MMSE on d. The diagnostic accuracy of T2J was estimated in the validation sample, the remainder of the TARCC cohort, and in the Japanese sample. The areas under the receiver operating curve (AUC; ROC) for T2J were compared in each sample, and against d in TARCC. The AUCs for T2J were statistically indiscriminable within TARCC, and in Japanese persons. In Japanese persons, AUCs for T2J were 0.97 for the discrimination between AD versus NC, 0.86 for AD versus MCI, and 0.79 for NC versus MCI. The AUCs for T2J in Japanese persons were higher than any individual psychometric measure in that sample. Valid d-score composites can be abstracted from a subset of delta's indicators. Moreover, those composites are exportable across cultural and linguistic boundaries.

    Objective: "delta", a latent variable constructed from batteries that contain both cognitive and functional status measures, can accurately diagnose dementia relative to expert clinicians. The minimal assessment needed is unknown. Methods: We validated 2 delta homologs in a convenience sample of elderly Japanese persons with normal cognition (NC), mild cognitive impairment (MCI), and dementia (n = 176). The latent delta homolog "d" (for dementia) was constructed from Instrumental Activities of Daily Living (IADL) and Japanese translations of the Executive Clock-Drawing Task (CLOX), Frontal Assessment Battery (FAB), and Executive Interview (EXIT25). The latent delta homolog "d3" was constructed from a restricted set of d's factor loadings. Results: d and d3 were highly intercorrelated (r = .97) and strongly related to both IADL and dementia severity, as rated blindly by the Clinical Dementia Rating Scale (CDR). d was more strongly related to IADL and CDR than any of its indicators. In multivariate regression, d explained more variance in CDR scores than all of its indicators combined. d's areas under the receiver operating characteristic curve (AUC) were 0.95 for the discrimination between Alzheimer's disease (AD) vs. NC, 0.84 for AD vs. MCI and 0.81 for NC vs. MCI. d3's AUC's were statistically indiscriminable. These AUC's are higher than any of d's indicators, as reported recently by Matsuoka et al. (2014), as well as the Mini-Mental State Examination (MMSE), which had been made available by Matsuoka et al. to the CDR raters. Conclusions: Latent variables can improve upon a battery's diagnostic performance and offer the potential for accurate dementia case-finding after a minimal bedside assessment.

    Background: The aim of the study was to develop the Japanese versions of Executive Interview (J-EXIT25) and Executive Clock Drawing Task (J-CLOX) and to evaluate the aspects of executive function that these two tests will be examining.
    Methods: The concurrent validity and reliability of J-EXIT25 and J-CLOX were first examined in all participants (n = 201). Next, the relationship between the two tests was examined using receiver operating characteristic (ROC), correlation, and regression analyses in healthy participants (n = 45) and participants with mild cognitive impairment (n = 36) and dementia (n = 95).
    Results: Satisfactory concurrent validity and reliability of J-EXIT25 and J-CLOX were shown. ROC analysis indicated that J-EXIT25 and J-CLOX1 were superior to the Frontal Assessment Battery, but inferior to the Mini-Mental State Examination (MMSE), in discriminating between non-dementia and dementia. J-EXIT25, J-CLOX1, and J-CLOX2 scores were significantly correlated with age, scores on the MMSE, Instrumental Activities of Daily Living (IADL) and Physical Self-Maintenance Scale (PSMS), and care level. In stepwise regression analyses of IADL scores, MMSE and J-EXIT25 were significantly independent predictors in men, and MMSE, age, and J-CLOX1 were significantly independent predictors in women. J-EXIT25, MMSE, and J-CLOX1 were significantly independent predictors in stepwise regression analysis of PSMS scores, and J-EXIT25 was the only significantly independent predictor in stepwise regression analysis of care level.
    Conclusions: J-EXIT25 and J-CLOX are valid and reliable instruments for assessment of executive function in older people. The present results suggest that these tests have common and distinct psychometric properties in the assessment of executive function.

    Objective: Because of the growing need for quick cognitive screening tests to distinguish Alzheimer's disease (AD) from mild cognitive impairment (MCI), we compare the diagnostic performance of a combination of the Mini-Mental State Examination (MMSE) and a Clock Drawing Test (CDT) to the Japanese version of the Alzheimer's Disease Assessment Scale-cognitive subscale (ADAS-J cog) in differentiating between patients with AD, patients with MCI, and healthy controls (HC). Methods: Data from 146subjects with AD and 60 subjects with MCI, as well as 49 HC, was retrospectively analyzed. We used logistic regression analysis with diagnosis as dependent variables and scores of the MMSE, the CDT-command, and the CDT-copy as independent variables, and receiver operating characteristic analysis to distinguish patients with AD from patients with MCI or HC. Results: When patients with AD were compared to HC, the independent predictors of AD were scores on the MMSE and the CDT-command. This combination was more sensitive than the MMSE alone and has nearly the same sensitivity and specificity as the ADAS-J cog. When patients with AD were compared to patients with MCI, the independent predictors were the MMSE and the CDT-copy. This combination was more sensitive and specific than the MMSE alone and was almost as sensitive and specific as the ADAS-J cog. Conclusion: The combination of the MMSE and the CDT could be a powerful screening tool for differentiating between patients with AD, patients with MCI, and HC. Its sensitivity and specificity are comparable to ADAS-J cog, which takes more time. © 2013 Kato et al, publisher and licensee Dove Medical Press Ltd.

    Objectives: In recent times, there has been an increase in the number of patients with type 2 diabetes, which has become a major social concern. The purpose of this study is to obtain the basic data that can assist us in developing useful psychological support for diabetics. Subjects and Method: Seventy-one patients with type 2 diabetes were administered the Problem Areas In Diabetes (PAID) scale, and forty-nine of them were administered the Profile Of Mood States (POMS) and the Tokyo University Egogram, New Version (TEG). Results: We eliminated 6 of the 20 items in PAID that were likely to cause floor effects. Factor analysis of the remaining items revealed one factor: "the diabetes-related burden." Further, through multiple regression analysis, we investigated the relationships between the PAID scale, which we developed using items that were contributable to this factor, and the subscales of the POMS or the TEG. The results showed that the PAID scale was affected by the "Fatigue" (F) subscale of the POMS. In addition, we classified high and low groups in the total score of the PAID scale and compared the mean of the POMS T-scores of the subscales and the mean of the subscales of the TEG in each of the two groups. We found that the responses of subjects in the high-PAID groups were significantly stronger in the "Tension-Anxiety" (T-A), "Depression-Dejection" (D), and "Confusion" (C) subscales of the POMS. On the other hand, there was no relationship between the PAID scale and the TEG. Conclusion: The result of this study suggests a strong relationship between the PAID scale and the POMS. The diabetics with high scores on the PAID scale tended to have sufficiently high emotional burdens to necessitate psychological support. Therefore, the PAID scale can be used in conjunction with the POMS in order to be able to understand in detail the state of mind associated with diabetes-related burden and provide psychological support for diabetics.

    In diabetes treatment, it is necessary for patients to control their weight. In this study, we conducted a research on the personality traits of obese patients with type 2 diabetes. The subjects consisted of 38 outpatients, and the following survey instruments were administered : the Tokyo University Egogram (TEG) New Version, the Profile of Mood States (POMS), and the Problem Areas in Diabetes (PAID) scale. Classified into the obesity group were male patients with a body mass index of 25+ as well as a waist circumference of 85 cm +, and female patients with a body mass index of 25+ as well as a waist circumference of 90 cm+. The obesity group comprised 23 patients, while the non-obesity group comprised 15. We analyzed the result of TEG, POMS, and PAID, which indicated that it was difficult for the obesity group to make plans or take an objective view of matters. They also tended to be emotionally less stable. This study suggests that it is important for the medical staff to fully consider the personality traits of obese patients with type 2 diabetes when motivating stress management by the patients themselves.

    Diabetics with serious somatic disease complications often lapse into a depressive state because of anxiety, anger, despair and remorse. We encountered a 58-year old man in a depressive state with serious somatic disease complications. We did non-structural interviews with the patient. In this study, we report the process of psychotherapy. In interviews, we found distorted cognition peculiar to the patient and his tendency to make extremely negative evaluation of his memories of past experiences. Therefore, we started searching intervention, in which we used reminiscences. The therapist gave him homework and suggestions because he became able to remember past situations objectively. The patient's chief concern changed from past to present after the intervention. As a result, the patient came to feel less negative about undergoing diabetic treatment. As the patient began to recognize his situation objectively, the therapist urged him on to recollect positive reminiscences. Thus, the patient was able to make proper revaluation of the past events. Consequently his depressive state improved along with recovering his self-esteem. In addition, we could discuss the improvement of this case from the viewpoints of both cognitive and narrative theories. This study suggests we can improve the depressive state and self-esteem of diabetics with serious somatic disease complications by helping them to recollect positive reminiscences with objective cognition.

    The purpose of this study is to obtain the basic data concerning the emotional function of persons with dementia. The subjects were 10 normal elderly persons and 10 inpatient elderly persons with mild Alzheimer's disease. They were requested to take the Mini-Mental State Examination and Emotion Recognition Test. Then, we statistically verified significant differences between the data of these two subject groups. The statistical analyses revealed that the reaction times of the mild Alzheimer's disease group were slower than those of the normal group in the case of happy and neutral emotional expressions. Further, the error rates of the misunderstanding anger as a sad emotional expression in the Alzheimer's disease group were higher than those of the normal group. These results were believed to indicate the possibility of the existence of conditions peculiar to mild Alzheimer's disease. It is important for future studies to relate these results with those of brain functional imaging studies.

    In this study, we examined the relationship between the severity of dementia of Alzheimer type (DAT) and mental dysfunction in patients with DAT In addition, psycho-social approaches in the care of dementia were discussed The Hasegawa Dementia Scale-Revised (HDS-R), Nishimura Dementia Scale (NDS), and Mini Mental State Examination (MMSE) were performed in 150 patients with DAT, and the passing rate of the subordinate items was investigated The results were evaluated using the Nishimura mental state scale for the elderly (NM scale) in 3 patient groups with borderline, mild, moderate, or severe DAT In the borderline and mild DAT groups, subordinate items showing a low passing rate included : orientation (date), 0 23 ; attention and calculation (serial 7's to 86), 0 37 ; and attention and calculation (four digits span, reverse), 0 37 In the moderate DAT group, the subordinate items showing a low passing rate were : attention and calculation (serial 7's to 86) ,0 15; verbal fluency (vegetables) ,0 18; and copy design (a pair of intersecting pentagons), 0 20 In the severe DAT group, the subordinate items showing a relatively high passing rate were : confrontation naming (pencil), 0 37 ; registration (plant, animal), 0 34 ; reading, 0 32 ; and confrontation naming (watch), 0 31 These results suggested that orientation is impaired in an early stage while verbal fluency, attention and calculation, and construction are gradually impaired Even when DAT becomes severe, confrontation naming and repetition appear to be relatively well maintained In the care of dementia, it may be important to give consideration to the characteristics of mental dysfunction according to the severity of dementia In the training of cognitive function, early intervention may be effective In patients with moderate DAT, programs that attract their attention such as those with game elements are useful In patients with severe DAT. aooroaches that aooeal to their maintained sensation can be used

    Empirical long-term research on rehabilitative training to recover functionality in daily life for dementia patients by recreational activities, creative activities and art activities has not been performed frequently enough. This study investigated features of the actual effects on the mental functions of dementia patients who had taken rehabilitative training to recover functionality in daily life for three years. Moreover, differences in functional recovery between DAT and VD after a three-year program were examined. As a result, it was suggested that mental function disorder found in both DAT and VD at the early stage had recovered to some extent, but the process of improvement differed between DAT and VD.

    The clinical psychological studies on the old age are extremely few as compared with the ones on the early childhood and youth. The Baum Test as one of the personality tests has often been used for the purpose of the medical practice. However, the results of the test for aged people have not been included in the past research. Thus, we tried quantitatively to clarify the feature of the tree drawings by elderly people who suffered from heavy dementia. In addition, we examined the index in the tree drawings that seen closely related with dementia of serious degree by the multivariate analysis (Hayashi's second method of quantification). We found that the analysis items concerning the accuracy of perception and acknowledgment (recognizing the trunk, branch and tree) were useful as an index which gave a degree of seriousness of the dementia. We also found that the item concerning the level of the desire and energy (size of drawing) was useful as an index which judges the degree of dementia's serious illness.

    The Mini-Mental State Examination (MMSE) developed by Folstein et al. (1975) is a simple test for several cognitive functions. However, the Impediments distinction standard has not been shown as for both original editions and existing Japanese versions yet. In this study, we have tried to standardize the Revised Japanese version of the MMSE-Aino (MMSE-A). Then, the distinction standard of cognitive function impediments was applied to MMSE-A, and the following result was obtained : the mean scores of MMSE-A were 4.6±4.1 in seriously cognitive impairment group, 11.2±3.8 in medium cognitive impairment group, 16.9±3.5 in slight cognitive impairment group, and 19.1±2.4 in borderline cognitive impairment group, respectively. It was recognized that there was a significant correlation between the scores of MMSE-A and the scores of HDS-R (Hasegawa Dementia Scale-Revised), and also between the scores of MMSE-A and the scores of NDS (Nishimura Dementia Scale), indicating the concurrent validity of MMSE-A was very high. Moreover, the test-retest reliability of MMSE-A and the internal consistency were very high.

    The cause for schizophrenia has been studied by biological, clinical molecular genetical, psychological, and social psychiatric researches. Although considerations on the problem from a clinical-psychological point of view can also be seen a little, detailed reports of psychological tests are still very few. Moreover, the study on twins has usualy been carried out after zygotic diagnosis. But the blind analysis without other information becomes important at a clinical scene in order to examine the clinical validity of psychological tests. This time, a study has been carried out on the premorbid character, the state of disease and zygocyte with only the result of the psychological tests of the twins whose zygotic diagnosis was not given. Later, this case has been diagnosed as the monozygotic twins by DNA finger printing method.

    高齢者数の増加に伴い、認知症により同意能力が低下した高齢者に対してどのように医療を提供すべきかが、医療を受ける側、提供する側双方において大きな課題となっている。 本プロジェクトでは、認知症高齢者においても、医療、福祉、行政の多職種によるサポートを受けてスムーズな医療選択および受療行動が可能になるためのシステムの開発を目的としている。具体的には、同意能力判定ツールの作成と、医学、法学、心理学の専門家、行政、高齢者、認知症高齢者とその家族が参加し、意志決定プロセスモデルを開発し、病院や地域で実際に運用してフィードバックを得ることで、現場で運用可能なシステムを構築する。
