BACKGROUND: The benefits of nationwide screening and tailored health guidance on improving obesity and cardiovascular risk factors is uncertain. AIM: To investigate the association of the national health screening and tailored health guidance with population health outcomes. METHODS: A fuzzy regression discontinuity design analyzed data of men and women aged 40 to 74 years who participated in a nationwide health screening programme in Japan from Apr 1, 2008, to Mar 30, 2018 and were recorded in the Japanese National Database. Exposure was assignment to the national health guidance of counselling on healthy lifestyle and clinical follow-up for individuals found to have waist circumference ≥85 cm for men ≥90 cm for women with one or more cardiovascular risk factors during annual national health screening. The primary outcomes were changes in obesity status and cardiovascular risk factors one year after screening. RESULTS: Of 3 490 112 men and 2 328 929 women, the assignment to the health guidance resulted in small reductions in obesity parameters: waist circumference; men, -0·27 cm (95% confidence interval [CI] -0·29 to -0·26); women - 0·34 (-0·41 to -0·27); body mass index, -0·07 kg/m2 (-0·075 to -0·066); -0·11 kg/m2 (-0·13 to -0·10); weight, -0·21 kg (-0·22 to -0·19); -0·28 kg (-0·32 to -0·24) that attenuated over time. Short-term improvements were also observed in blood pressure, haemoglobin A1c, fasting glucose and triglycerides across both sexes. CONCLUSION: A nationwide health screening programme was associated with only small, and transient improvements in obesity and cardiovascular risk factors.

This study aims to evaluate the impact of intergenerational transfers on the budgetary constraints of adult children in Japan by using the Engel method to estimate the economic burden or benefit represented by their elderly parents, whose socioeconomic heterogeneity is kept into account. Low-income elderly parents put some strain on their sons and their sons’ families: the study’s (admittedly rough) estimates indicate that a middle-aged family would have to earn about 12% more in order to preserve the same living standard of an otherwise comparable family, but without low-income elderly parents. In contrast to this, the wife’s high-income elderly parents are beneficial to the family budget: the order of magnitude of their help, when they are co-residing, is of about 24% of the (daughter’s family) income.

This paper identifies the determinants of income inequality in Thailand, which increased between 1975 and 1998. Our regression results strongly suggest the significance of agricultural factors. We also obtain some limited evidence that sectoral factors, financial development, and education level disparities play a roughly equally important role in explaining inequality changes in Thailand. This paper analyses the optimal control of government securities. The special role of government securities under aggregate macroeconomic shocks results from the government's ability to collect taxes. Liquidity is loosened by an increase in the supply of government securities. The policy-maker, however, faces a trade-off between an ex ante incentive for entrepreneurs to expend effort and an ex post efficient utilization of investment opportunities.

In this paper modified versions of the Balassa-Samuelson model are developed. We stress the effects of balanced productivity growth and capital accumulation, which is often ignored or not fully understood in existing studies. Our theoretical analysis shows that these effects and the differential productivity growth effect can be presented in a unified framework. Empirical estimates using 1970-1990 sectoral data for the OECD show that our modified models are more suited to the data than the commonly used Balassa-Samuelson model. Corporate investment is the most important factor to explain the long stagnation of Japan during the 1990s. Using the Bank of Japan diffusion indices of "real profitability" and "banks' willingness to lend," we estimate investment functions for four groups of firms: large/small and manufacturing/non-manufacturing. Our results suggest that for large firms, financing constraints are not significant whereas the converse is true for small firms. A fall of investment during 1992-94 is largely explained by real factors. However, the credit crunch occurred beginning 1997 and it lowered the growth rate of GDP by 1.6%. (C) 1999 Academic Press. (1) We conducted a randomized controlled trial (RCT) focusing on how knowledge of sexually transmitted diseases, represented by HIV, can be acquired. (2) We conducted a web-based questionnaire and the web leaflet. We divieded respondents into the leaflet reading and non-reading groups to determine whether increasing knowledge about sexually transmitted diseases and becoming aware of the high number of infections in their generation would lead them to avoid risky sexual behaviors or get tested in an RCT. The results showed that (1) distributing a leaflet describing sexually transmitted diseases and having the participants peruse it for a certain period of time increased their knowledge more than those who did not receive the leaflet, and (2) knowledge increased only among young women, but did not lead to behavioral change.
